Terms and conditions for 12 Months Unlimited
Between: ______________ and CrossFit 2024 for CrossFit - 12 month agreement
Contract Duration and Automatic Renewal: Contract is in force for 12 months from the day contract is signed, with automatic renewal for subsequent months, unless cancelled by either ____________ or CrossFit 2024, with notification of 30 days in advance of the next billing date.
Agreement to Pay Recurring Fees: ___________ agrees to pay CrossFit 2024 monthly recurring fees as follows: Unlimited classes - 12 Month Commitment for $185.00 via Client authorized automatic credit card or ACH.

(a) I agree to purchase the Unlimited classes - 12 month commitment for $185.00, as an automatic charge to my credit card, or automatic debit to my checking account each month for a contract period of 12 months.
(b) I hereby certify that I am the holder of the credit card, or an authorized signer on the bank checking account detailed below.
(c) I understand that I will be notified if my credit card or checking account payment fails to authorize for any reason, and that a $10 late fee will apply if I do not provide a valid credit card or checking account ACH information within 10 calendar days of the original rejection date. (d) I understand that my service will be deactivated if my account becomes more than 30 calendar days late.
(e) I understand all Membership Cancellation Policies and Membership Hold Policies.
Membership Cancellation Policies:
In the event that you want to cancel your membership, a written notice must be given 30 days prior to the next billing cycle. Cancellation fees will be charged as follows:
Pay the difference between the agreement price purchased and the agreement price that should have been purchased according to length of membership.
Membership Hold Policies:
To put your membership on hold and freeze Auto-Pay, a completed request must be submitted in writing at least 5 days in advance of your next billing date in order for your request to be effective from the following month. Standard cancellation policies apply if you decide you need to cancel membership while on hold. You are allowed 1 (one) Freeze per year for 30 days. A hold fee of $25 will be taken upon hold placement.
I understand if I have an unpaid balance to CrossFit 2024 and do not make satisfactory payment arrangements, my account may be placed with an external collection agency. I will be responsible for reimbursement of the fee of any collection agency, which may be based on a percentage at a maximum of 35% of the debt, and all costs and expenses, including reasonable collection and attorney’s fees incurred during collection efforts.
In order for CrossFit 2024 or their designated external collection agency to service my account, and where not prohibited by applicable law, I agree that CrossFit 2024 and the designated external collection agency are authorized to (i) contact me by telephone at the telephone number(s) I am providing, including wireless telephone numbers, which could result in charges to me, (ii) contact me by sending text messages (message and data rates may apply) or emails, using any email address I provide and (iii) methods of contact may include using pre-recorded/artificial voice message and/or use of an automatic dialing device, as applicable.
