Between: ____________________ and CrossFit 2024 for CrossFit - 12 month agreement

Contract Duration and Automatic Renewal: Contract is in force for 12 months from the day contract is signed, with automatic renewal for subsequent months, unless cancelled by either ____________ or CrossFit 2024, with notification of 30 days in advance of the next billing date.

Agreement to Pay Recurring Fees: ___________ agrees to pay CrossFit 2024 a one time non-refundable fee as follows: Unlimited classes - 12 Month Commitment for $2100.00 via Client authorized automatic credit card.

(a) I agree to purchase the Unlimited classes - 12 month commitment for $2100.00 (non-refundable), as an automatic charge to my credit card, or automatic debit to my checking account one time for a contract period of 12 months.

(b) I hereby certify that I am the holder of the credit card, or an authorized signer on the bank checking account detailed below.

(c) I understand that I will be notified if my credit card or checking account payment fails to authorize for any reason, and that a $10 late fee will apply if I do not provide a valid credit card or checking account ACH information within 10 calendar days of the original rejection date. (d) I understand that my service will be deactivated if my account becomes more than 30 calendar days late.

(e) I understand all Membership Cancellation Policies and Membership Hold Policies.

Membership Cancellation Policies:

In the event that you want to cancel your membership, a written notice must be given 30 days prior to the next billing cycle. Cancellation fees will be charged as follows:

Pay the difference between the agreement price purchased and the agreement price that should have been purchased according to length of membership.

Membership Hold Policies:

To put your membership on hold and freeze Auto-Pay, a completed request must be submitted in writing at least 5 days in advance of your next billing date in order for your request to be effective from the following month. Standard cancellation policies apply if you decide you need to cancel membership while on hold. You are allowed 1 (one) Freeze per year for 30 days.

This pre-sale membership fee is non-refundable for any reason.
